Purpose: This rental is to provide the Renter with a NeurOptimal® system that will permit the Renter to provide NeurOptimal® Sessions to him/herself, family, and friends. The Renter may share the costs and use of this equipment with another, but the person signing this contract accepts full responsibility for the safety and return of the equipment.

Cost: On loan for unlimited in-home sessions. It is recommended that you do no more than two Sessions in a day per user with at least 2 hours between sessions. Fees are due prior to pickup. There is a $50 fee per day for overdue system returns. Failure to return the equipment within 15 days following the termination of this contract will result in the borrower's credit card being charged $9,495 as payment in full for the system. At this point the borrower becomes a full legal owner of the license and equipment with all privileges thereto. These fees are separate and additional to the contracted rental rates above.

Duration: This rental agreement runs from the date of system pick-up until the agreed return date. If the borrower wishes to return the equipment prior to completion of the contract, there will be no refund for unused time.

Note: If the Renter purchases a NeurOptimal® system exclusively with Brainflow, the Renter will receive $300 as a token.

Limitations: Owner guarantees equipment functionality; however, we cannot guarantee the types, degrees, and order of shifts the Renter and anyone else who uses the system may or may not experience. Completion of Training Tracking Tools is recommended.

Care of Equipment: The equipment being rented remains the property of Brainflow. The Renter accepts personal responsibility for the care of the equipment. Renter will log in for technical support at the Owner's request if needed to troubleshoot the system; there will be no charge for this service. No alterations, work, or additions may be made to the computer at any time except by Zengar Institute technical staff. In the event of damage, theft, or loss, the lender must be notified immediately. Compensation for lost, damaged, or stolen equipment is the responsibility of the borrower, and therefore insurance for the equipment is the responsibility of the Renter.

Total value of this system: $10,995 (Sensors $145, Tablet Replacement $1,795, zAmp $1,950, plus any applicable shipping costs).

Internet Connection and Windows Updates: Borrower understands that an Internet connection is required for NeurOptimal® to run and agrees to regularly perform Windows updates while this unit is in their possession.

I hereby agree to return the rental unit one month after system pick-up/arrival, and I understand that a $50 per day fee will be due if the system is not dropped off or shipped by the due date unless a rental extension has been arranged with the Owner. Renter reserves the right to request an additional rental term and will receive priority regardless of demand.

I understand that my credit card will be kept on file during the rental term(s) and will only be charged in the event of loss, theft, or damage.
